Financial

We are here to help you receive the treatment you need and want. We do not file any insurance in the office; however, we are happy to provide you with the necessary insurance forms needed for reimbursement from your insurance carrier.

Forms of payment accepted by our office are check, cash, and most major credit cards.

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for a scheduled appointment without sufficient notice.
